A bug introduced in an Old School RuneScape update caused players to lose items after dying, prompting Jagex to take game worlds offline for repairs and reissue items to affected players. The bug skipped steps in the post-death item retrieval process and deleted players' items instead of storing them, according to GamesRadar.

Jagex confirmed the issue and warned players to avoid PvM content temporarily. Some players were punished with two unsafe deaths after mistakenly respawning inside boss arenas and being killed twice.

After taking the worlds offline, Jagex reissued items to the players who had been affected.

Death mechanics in the spotlight

Per the OSRS wiki, when a player dies outside safe zones, valuable items are stored in a grave marker for 15 minutes; if not reclaimed, they are sent to Death's Office for a larger fee. Some boss fights have their own item retrieval storage, and dying an unsafe death before collecting those items results in permanent loss.
