Ice Pick Lodge and HypeTrain Digital released The Great October Update for Pathologic 3 on PC and consoles on October 25, 2026, according to Bloody Disgusting. The patch is available on Steam, PlayStation 5 and Xbox Series.
The update adds a Photo Mode with weather presets, stickers and creative tools. Two new difficulty options, Easy and Hard, now sit alongside the existing Normal mode.
Steam Deck support arrives with the patch, along with performance improvements aimed at lower-end CPUs and ultrawide monitor support. In-game subtitles have been added for in-game voice-over, and the update introduces new ways to deal with the creature Shabnak.
Quality-of-life changes cover refined interfaces, controls, map navigation, audio and game balance. More than 100 fixes address dialogues, quests, story events and branching outcomes across the game's 12-day campaign.
The studio also published a behind-the-scenes video about the making of the update, which had been in development since early summer.
